Overview

A large range of procedures has been used over the last decades to help pain patients.  Some of these have been proven to effective and safe time after time when used in the right patients.  Others have been discarded as inefficient or too dangerous with regards to serious, sometimes life-threatening side effects.

Unfortunately it is difficult to determine the success rate and the risks associated with a particular procedure due to large differences in patient selection, how the procedure is performed, which drugs are used, how much of the drug is used, etc.

The ACC IPM intervention index attempts to give some guidance to the various procedures.  However, it is important to recognise that the opinions displayed within this index are indeed only opinions.  Please discuss any information you found on a particular procedure with your pain physician.  At times even world-renowned pain experts discuss bitterly the pros and cons of a particular procedure, which highlights that there may not be one single truth out there.  Learn about the procedure and discuss the procedure thoroughly with your doctor.

Most interventional pain procedures can be considered within the following categories.

Early intervention in neuropathic pain
(to prevent progression to chronic pain states)
Peripheral nerve blocks
Sympathetic blocks
Intrathecal Drug Delivery
Targeted Interventions in musculoskeletal pain (diagnostic) Zygapophyseal joint (medial branch or intra-articular)

Disc (provocation discography)
Targeted Interventions in musculoskeletal pain (therapeutic) Radiofrequency neurotomy
Intra-articular blocks
Intradiscal electrothermal annuloplasty
Implanted devices Intrathecal pumps
Neuromodulation (spinal cord, sacral nerve root and peripheral
nerve stimulation)
Tissue ischaemia Sympathetic blocks
Spinal cord stimulation
Cancer Pain Coeliac plexus block
Intrathecal analgesia
