Referrals/First Time Assessments

Referrals: Rotorua Pain Specialists Ltd is happy to receive referrals from all Health Care Professionals and Case Managers involved in the management of patients with chronic pain.
Funding: We recommend strongly that you as a patient assure adequate funding is available. Please obtain prior approval by any insurance company or institution which you believe is responsible for funding any assessment or treatment prior to you seeing any member of our team. Failure to do so may leave you with a private bill which you and your family will have to cover.
Pain Questionnaire. We will not be able to see you without you having filled out our Pain Questionnaire. Please take your time to fill out all questions. We want to know as much as possible about your pain and about other factors which may play a significant role in trying to help you. Too much time is wasted otherwise in trying to find out simple facts I require to create a management plan for you. Send the Questionnaire back to PO.Box 12083, Rotorua.

Please ask your GP or a Specialist to refer you to us. Your GP will be able to shed light on other medical problems you suffer from and will easily be in the position to tell us exactly what she / he has already tried to help you with your pain. Most of our recommendations can be followed by your GP after the initial assessment and essentially it is your GP who does know you best, compared with most other medical practitioners involved.

Please ask you GP or your Specialist for a referral. Once we have received your referral we are able to see you without needing aprior approval by a case manager. This is however only true if you have a valid claim number linked to the pain you are currently suffering from. Please give us a ring prior to any appointment to discuss any uncertainties to avoid you having to fund the fee for the assessment yourself.

Unfortunately we can not see any patients that are not covered by ACC or who can not finace their assessment themselves.
